Request: close request

As a coordinator I want to close the Request for help when the help ended for whatever reason so I can keep organized.

Definition of Done:

• Request can be closed either from list or from its detail
• When invoking the close request action the close form is displayed (preferrably modal window)
  • User must select whether the request was fulfilled, failed or became irrelevant (see entity model)
  • note can be added with the reason why it being closed
  • date and time is captured in closed_at field
  • user is captured in closed_by_id field
  • request state is set to closed
