PHP code shouldn't appear inside a css or img folder, for example.
When doing a VIP theme review, placing PHP or JS files inside folders where they're not expected can lead to code getting skipped; for example if a PHP file was buried 4 levels deep in an img hierarchy.
Shouldn't happen in practice, but let's enforce it.
