Simplify Mental Model For "Create Volunteer Family" UI

1. Simplify/clarify/improve mental model
   If we are creating a new family using a new adult as a starting point... what does "Relationship to Family" mean? For example...

• when I'm filling out this field, am I saying what my own relationship is to the family, or am I saying what the relationship is of the first adult to the remainder of the yet-to-be-entered family members?
• Why is "In Household" a necessary checkbox?
• • Is "household" meant to mean a singular geographic location or another naming convention for a family unit?
• • Are we asking if the first adult lives with the other members of the family that haven't been entered yet?
• • Are we asking if the first adult is even a member of the family (i.e. if the first adult happened to be a live-in family friend)?

2. Once there's a clear understanding of how the various pieces of information tie together (both to each other & to the user filling them in), go through & make the following updates to the UI:

• Simplify title to 2-4 words
• Remove and/or collapse fields that are not crucial to the initial creation process and ensure that that data can instead be added/edited at a later point if needed

Fields we may especially want to consider removing

• "Ethnicity"
• "Mobile" vs. "Home" distinction for phone number (are we sending sms? are we expecting improved reachability?) - depending on how this is answered, there may be ways to improve the UI
• "Address 2" (Replace with "Add Address Line", which could then lead to "Address 3" for military/PO situations)
