[Analytics Infra] Synchronize the Index Address Book #166

There's a user-generated table that defines the Index address book, but we should synchronize it with this:
https://github.com/IndexCoop/address-book

and that way we have a single source of truth.

We should also recreate that user generated table using the Index Coop dune account
