This was caused by me doing this:

Same here, also with a bookmarked image.
Nice to see that 2 years have been past and this bug was still not fixed.

This is a very annoying issue :/ I have bookmarked several image captions (selected the caption - add bookmark) and somehow trying to update references often re-applies that bookmark to contain the entire image. Then this error occurs, bookmark must be deleted and all references re-linked which is a real pain in a 120+ page long document with many headers and 80ish figures.

Also, some of the bookmarks are simply removed when this error is thrown..
