Headings not saving/saving incorrectly

• Contacts - displaying author as Tony Shannon - not Test User
• MDT is not recording the following fields
  - Questions for MDT
  - Meeting discussion
• Procedure date - not recording correctly. I enter a date and it defaults 10/6. system information date also missing
  - Procedure Performed by - defaults to Performer even if i enter a different value
• Transfers of care not saving/storing nor are referrals entries available for selection

we have a separate issue for TOF, as we worked only on frontend part: #20
