Let's introduce some breaking changes in the new version of fhir-sdc

Written in TS to be reusable between frontend and backend TS implementation discussion (fhir-sdc v2) #65
It should work with FHIR format in expression evaluations Pass FHIR instead of aidbox format as context and env for fhirpath expressions (fhir-sdc v2) #66

We have quite long list of open issues, I'll try to go through them and prepare list of backward incompatible but important changes
