If a template is disabled, and you have permission to add it, flag it as disabled

ie I intentionally disabled a template so it would not show in the picker, but then it showed anyway because I'm an admin. But these are real clutter. I can sense that in the future we might want to have a harder disable flag which is disabled for everyone when adding new ones.

In the interim when adding it it would be good to have something like '[DISABLED] Learning outcomes' in the picker so its more obvious
