Same as vanilla Minecraft with respect to crossbows; unique to the Bow of Lost Souls with respect to bows. For the secondary arrow to fire, there must be multiple entities around as the bow to fire the second arrow at a secondary target. The idea behind this is that the second arrow is being fired by the souls of the bow, not by the player.
Attacking drains your life to grant one stack of Pain Cycle. At 5 stacks, your pain is channeled into your next attack.
