Instead of registering, people must request to join the community.

User record is created in the database; but has a field approved set to false
Logged in users don't see Request to Join link
People who request to join must not be able to log in until they are approved

@Wimsy113 I'm OK with the user specifying their password when they register; but they can't be able to log in until they're approved.
