Sorry for the delay.

It's explained in the README.

You have to :

add UserParameters (wireguard.conf for example),
create a discovery script (zabbix_agentd.d/wireguard by default),
permit execution for user zabbix via sudo.
import the template in Zabbix.
add template to hosts owning the WireGuard app.

All files are given in the git repo.
