Update documentation

Observed behaviour

I changed a lot of code without changing the documentation, which is bad. Especially the read-the-docs stuff.

Expected behaviour

Documentation should reflect current codebase.

#4545 does some of this but not all; I want the ReadTheDocs site to do something more clever with the profile modules.
