Templates for newly filed issues and PRs

Consider adding GitHub templates to add guides for users filing issues and PRs. This will help introduce new users, encourage best practices, and get more consistent code quality.

If all else fails, I've the necessary permissions to enable this, but it should be possible for @brews to do this, too.
