Allow prepopulating a default prometheus address

Currently, end-users can provide a URL to an existing prometheus address when installing kots to an existing cluster. However, some vendors may want to optionally ship prometheus within their kots app, and have this automatically be setup. So we should add a field to the app spec (or elsewhere) to allow changing the default prometheus address to a vendor-supplied value.
