When turning off multi-column table of contents, the multi-column environment is undefined, this is done on the example.tex.

Sorry that I am just getting to this. Does this error occur for you with example.tex as it is distributed? If so, please describe your environment, version of LaTeX, etc. If not, please post an MWE that demonstrates the error.
