Add affiliation metadata field for creators and collaborators #559

Datacite metadata schema has this field (so adding this field to Hyku would comply with our policy to match the Datacite metadata schema).

BL Scholarly Comms were interested in knowing where research collaborators are based. Adding this field would enable such reporting.
