To prevent bad things from happening to firehose (like having 4000 connections go to a single firehose endpoint), add a new configuration parameter to firehose grpc endpoint that works similarly to substreams-tier2-max-concurrent-requests, in that it returns an error when limit is reached. Default can be unlimited like now.
