The survey link is broken and the CoC opens the markdown file rather than the compiled page.

I've got a fix for it in my fork, I could open a PR if that's ok @steely-glint.

Sure - please fire up a PR -
One of the things I'm struggling to keep track of is the difference between the GitHub and the public site from GitHub pages - the markdown renders differently in the two spaces. Sigh.
