I have added the VST, and added plugin
but unfortunately, I do not see any difference in the background noise.

not sure which one is turned ON, one where inputs are disabled or when enabled. I tried both worked with none

I also had the same thing, figured I was just doing something wrong and gave up. Have not used EqualizerAPO before (yes after installing it for the mic I did restart)

Very hard to tell what is on/off but I tried both extensively.

I see that the latest version of Equalizer APO is 1.4. Updating it probably wouldn't hurt.

Also, what if you select stereo plugin?

@Rayregula yes it is hard to tell when it is On /Off

Probably something funny going on with channels. Seems like laptop mic, I'm not sure what could be wrong aside from maybe it having more channels than two, then mixing them in some way.

I had the same issue and I decided to just change this one setting in the Microphone Properties > Advanced Tab.
Try enabling signal enhancements and see if it works for you. For some reason when I tested it right after enabling it, it started working perfectly.

I'm also having the exact same problem, sometimes I get it to work but if I restart my PC it just doesn't work anymore.
Update: I found the solution!
Go to rnnoise_stereo.dll > Properties > Security > Edit > Add > Type in NT SERVICE\Audiosrv > Grant Read & Execute Access > Restart

Okay it is not working again I give up lol. Went back to using v0.91.

*Second update: This is probably my last update, after spending hours on Process Monitor I saw that audiodg.exe was trying to create 2 files dxgi.dll and wininet.dll in a folder called "ElevocInstallDriver" but got "NO NAME FOUND". Then I used DriverStoreExplorer to uninstall every drivers with the name Elevoc in it and the plugin worked (again).

I have no such section.

Go to rnnoise_stereo.dll > Properties > Security > Edit > Add > Type in NT SERVICE\Audiosrv > Grant Read & Execute Access > Restart

Doesn't work for me too.

It was also not working for me. But eventually I got it to work. Here is what I did.
I installed the latest 64 bit version and only used the VST plugin, removed all the other stuff like the preamp and eq.
Also like @lyshoicakes mentioned I made sure Audio Enhancements are enabled on my microphone.
