Changing the number of sets in custom exercises duplicate the form #1553

Steps to Reproduce

Create a new exercise
Create a new workout and add that exercise
Change the number of sets

Expected results:
The form should only show the numbers of sets selected.

Actual results:
The new form with the selected number of sets is append after the last form.
