Add instructions for extending xmpp-ftw to the /extend endpoint.

Involves:

Add a route for /extend
Add links in the footer and main index page
Write documentation*
Its best to look at one of the simpler modules to give an idea of how to do this, e.g. see https://github.com/xmpp-ftw/xmpp-ftw-ping
