Online application form for the ILS Institute

Introduction

This application is used when a student gives their details and it will be converted to pdf format. It won't be saved in the database.

Gender and date of birth will be generated automatically when the user enters their NIC number.

If the user provides the incorrect NIC number, the application cannot be submitted.

Used Stack : HTML 5, CSS, Bootstrap, PHP, Ajax, jQuery, FPDF.
