Licensing exemption for CareTogether trademark

This commit updates the license to include an explicit exemption clause that clarifies the trademark status of the CareTogether name and logo as remaining the proprietary property of Coept LLC.

+             TRADEMARK LICENSING EXEMPTION FOR CARETOGETHER CMS
+
+  While CareTogether CMS is licensed under the GNU Affero General Public
+License v3.0 (AGPL-3.0), Coept LLC retains all rights to the CareTogether
+name and logo trademarks. The following additional permission is granted
+under the AGPLv3's section 7 to address the use of these trademarks:

+  Notwithstanding any other provision of the GNU Affero General Public
+License version 3.0, the use of the CareTogether name, logo, or any other
+trademarks associated with CareTogether CMS (the Software) owned by Coept
+LLC requires explicit written permission from Coept LLC. This permission is
+necessary for any use of the CareTogether trademarks that suggests
+endorsement by, association with, or sponsorship by Coept LLC, including
+but not limited to, the distribution, advertising, or marketing of software
+derived from CareTogether CMS.
