Remove flux2fw from build. This is no longer used, since both original and fwave Riemann solvers are handled from flux2.
Pass maux into Riemann solvers from flux2. Typedefs are not changed, however, since this would require changes to all 4.x applications.
Remove labeled do-loops from 4.x inline limiter.
