add # to SGV badges without options

For some reason, the RST rendering on GitHub doesn't like missing `#` in URIs for the badges. When they are omitted, there is a visual glitch from the link reference underline. Adding the `#` explicitly removes the visual glitch, and doesn't affect the rendered badge image.
