Spanning header updates

Multiple updates needed for spanning headers

• We currently only allow for a single spanning header, and we should allow for as many as are needed.
• We'll need to migrate the spanning header storage from .$table_styling$header to a separate structure that is more flexible for multiple headers.
• We'll also need to add functions for adding and remove footnotes to these spanning headers
