Added "online-always" mode to quickly download the latest NIH citation data

Added an "online-always" mode to always download the latest NIH citation data,
tripling the speed of the software by not writing data to temporary working files
for later offline use.

The default mode is now: "online-always."
The current "online/offline" mode is now made an option.
