Reject quantified bounds with non-constant refinement types #1114

This reintroduces a restriction lifted in PR #1112 until we have time to address it properly. The restriction was initially lifted to allow an existentially quantified index for a history variable to have a refinement type defining its range between zero and the current time. This PR applies a workaround to permit only that specific case.
