Fix duplicated environments in interpreter list (microsoft/vscode-pyt…

…hon#22964)

Failing tests are due to
microsoft/vscode-python#22965, this can still
be reviewed.

Closes microsoft/vscode-python#22918 closes
microsoft/vscode-python#22146
