This PR contains an initial version of the jupyterlab extension.

Approach
The extension was created using https://github.com/jupyterlab/extension-template, and lots of learning from the example extensions at https://github.com/jupyterlab/extension-examples.
Automated Tests
There are tests for the serverextension (back end). There aren't front end tests yet.

Make sure the agent is built and is on your PATH.

, since the plugin currently hardcodes this (until #185 or full account selection is implemented in the agent UI).Follow the install/build instructions in the README to get up and running.
When you open a notebook, you should see the Publish button with the blue icon on the right side of the notebook toolbar. Clicking it should open the agent UI in a JupyterLab panel. The publish button is functional, but you won't see progress feedback or logs until those are implemented in the agent UI.
